This is the "Adoro te devote", a song written by St. Thomas Aquinas sometime in the 1200s.
It amazes me, reading these lyrics, to see how the Eucharist humbled such a man of logic. He recognized that reason could only take you so far, and was willing to give his mind entirely over to God.
